Eligibility Criteria
- Adult (over age 18) resident of metro Atlanta
- Must be uninsured
- Income less than 200% of the Federal poverty level
- You must provide gross family earned and unearned monthly income and disclose family size (number of dependents only)
- Patients seen by appointment only. Appointments can be made by calling.
